Submitting Insurance Claim Forms

All claims are to be made in writing by using and completing all relevant details on this claim form. Original detailed receipts, bills and invoices you want to claim for payment are to be submitted with your claim form and mailed to the address located on the claim form. In addition, please attach medical records.
