Please identify yourself as a EurAupair International Program Participant and refer to the policy number found on your insurance card.
Submitting Insurance Claim Forms: All claims are to be made in writing by completing all relevant details on the claim form. Original detailed receipts, bills and invoices you want to claim for payment must be submitted with your claim form and mailed to the address located on the claim form. In addition, please attach medical records.
